By participating in ANY AND ALL activity, class, event on (or outside) the property of GENESIS STUDIOS located at 2861 W AVE 35, Los Angeles CA 90065 and/or organized by GENESIS STUDIOS, LLC or any third  party, and/or use of the property, facilities, and services of GENESIS STUDIOS, LLC in any capacity, I understand that I am agreeing for myself and (if applicable) for the member(s) of my family, to the following: 

RELEASE, ASSUMPTION OF RISK, MEDICAL RELEASE AND INDEMNIFICATION: I (“I”, “me”, “Participant”) HEREBY RELEASE, WAIVE, DISCHARGE, HOLD HARMLESS, AND COVENANT NOT TO SUE Mackenzie Martin, Inc. and/or GEOMETRY DANCE COMPANY, its directors, employees, agents, volunteers, or any student for ANY and ALL liability, suits, or judgments of any and every kind arising from any injury (physical or mental), any risks arising from negligence or carelessness, property damage, theft, or death that I may suffer as a result of my participation in the convention and/or showcase. I acknowledge that I am responsible for the cost of any and all medical and health services that I may require as a result of participating in the above-named convention. I understand there is an assumption of risk and great physical demand in participating in this event.

In the event of any medical emergency, I DO AUTHORIZE and CONSENT to all dental, medical, or surgical diagnosis or treatment, and hospital care that Mackenzie Martin, Inc. or GEOMETRY DANCE COMPANY’s directors deem necessary for my safety and protection. This Agreement shall be construed in accordance with the laws of the State of California.

SUPERVISION (DANCERS UNDER 18 YEARS OF AGE): Attendance at Mackenzie Martin, Inc. and/or GEOMETRY DANCE COMPANY is permitted only under the understanding that the Company is NOT responsible for supervision of participants during events, classes, or anywhere on or around the premises.

TALENT RELEASE: By attending the event, I willingly subject myself to areas where filming, production, photography, and videography are being used, and I consent to Mackenzie Martin, Inc. and/or GEOMETRY DANCE COMPANY using my likeness on social and marketing accounts. I assign and grant the right and permission to use and publish the photographs/film/videotapes/electronic representations made of me during the event, releasing the company from liability for such use.

COVID-19 (AND ALL OTHER VARIANTS) ASSUMPTION OF RISK: I acknowledge and accept the risk of contracting COVID-19 in order to attend Mackenzie Martin, Inc. and/or GEOMETRY DANCE COMPANY’s events and premises.

FEES/DAMAGES: I understand and agree to pay for all damages to the facilities of Mackenzie Martin, Inc. and/or GEOMETRY DANCE COMPANY caused by my actions or those of my family, agents, or extensions.

DISPUTE RESOLUTION: Any disputes arising out of this Agreement will be submitted to mediation or binding arbitration under the rules of the American Arbitration Association.

NO DURESS: I acknowledge that I am under no pressure to sign this Agreement and have had ample opportunity to review it or consult legal counsel. I also acknowledge that Mackenzie Martin, Inc. and/or GEOMETRY DANCE COMPANY has offered to refund any fees I have paid if I choose not to sign.

I HAVE READ THE AGREEMENT AND FULLY UNDERSTAND ITS TERMS. I AM AWARE THAT THIS AGREEMENT INCLUDES A RELEASE AND WAIVER OF LIABILITY, AN ASSUMPTION OF RISK, AND AN AGREEMENT TO INDEMNIFY THE RELEASEES.
